Confidentiality and Release of Information
Information regarding a client will not be released to anyone without written permission from the client. In some cases, and in for specific circumstances, exceptions may apply. Counselors may break confidentiality by releasing information to proper authorities if the student:
  • Demonstrates a clear and imminent danger to self or someone else.
  • Indicates that he/she was abused.
  • Directs via written request that information should to be released to a specific party or parties.

Client files are not to be copied and released to the client or anyone else. Upon the client’s written request, a summary letter regarding treatment and progress notes will be prepared by their counselor. In some cases, actual case notes must be made available. This might be the situation with court/litigation related matters.
